Topical CBD For Joint Inflammation – How Much Do You Need?

So many people want to use topical CBD products like cremes, salves, lotions, etc… for joint inflammation but have no idea how much to use. Everyone’s body is different, and everyone metabolizes it differently. However, a recent study dug into this on a more in depth analysis.

This study by the National Institutes of Health examined the efficacy of transdermal CBD for reduction in inflammation and pain. It assessed any positive effects, as well as adverse effects in adjuvant-induced monoarthritic knee joint models. The rats had joint circumference and immune cell invasion measured to help indicate the level of inflammation.

So, what did it find? Most importantly, it was shown that CBD significantly reduced joint swelling, limb posture scores, “immune cell infiltration, thickening of the synovial membrane in a dose-dependent manner.” Analysis of the spinal cord and dorsal root ganglia revealed dose dependent reductions of biomarkers for inflammation.

How much CBD were these rats given? Results showed that 6.2 and 62mg/day were effective doses. Interestingly, The inflammation was not reduced significantly with higher doses of CBD. The data indicated that “topical CBD application has therapeutic potential for relief of arthritis pain-related behaviours and inflammation without evident side-effects.”

So, what this means is that they found the optimal dose for CBD is 6.2mg’s per day to significantly reduce the inflammation that was previously present. Amazingly, almost 50 million (22.2%) of adult Americans over 18 were diagnosed with arthritis between just 2007 and 2009. By 2030, the Centers for Disease Control and Prevention, are predicting an increase to 67 million.

Although this evidence is anecdotal, there is definitely promise for those struggling with rheumatoid arthritis, and osteoarthritis – the most commonly diagnosed types of arthritis.

A lot of people are curious as to how CBD can impact their bones. It turns out, that anecdotal research shows that it may impact them dramatically – at least in mice. According to this study, the endocannabinoid system, has been implicated in the regulation of bone metabolism.

Cannabinoids interact with type 1 (CBD1 and CB2) cannabinoid receptors and the GPR55 receptor to regulate cellular function through various signaling pathways. The study points out that endocannabinoids are actually produced in bone, but it is unknown how the mechanisms regulate their production work.

“Mice with CB(1) deficiency have high peak bone mass as a result of an osteoclast defect but develop age-related osteoporosis as a result of impaired bone formation and accumulation of bone marrow fat. Mice with CB(2) deficiency have relatively normal peak bone mass but develop age-related osteoporosis as a result of increased bone turnover with uncoupling of bone resorption from bone formation. Mice with GPR55 deficiency have increased bone mass as a result of a defect in the resorptive activity of osteoclasts, but bone formation is unaffected.”

The study goes on to say that cannabinoids are also produced within synovial tissues (a specialized connective tissue that lines the inner surface of capsules of synovial joints and tendon sheath. It makes direct contact with the synovial fluid lubricant, which it is primarily responsible for maintaining.) Preclinical studies show that cannabinoid receptor ligands are effective in treatment of inflammatory arthritis. “These data indicate that cannabinoid receptors and the enzymes responsible for ligand synthesis and breakdown play important roles in bone remodeling and in the pathogenesis of joint disease.”

Restless Leg Syndrome – Can CBD Help?

Dr. Allan Frankel, MD,  of GreenBridge Medical wrote his experience with CBD and restless leg syndrome  with his patients. Some people question whether Restless Leg Syndrome (RLS) is real – Dr. Frankel says it is, and that “millions of Americans suffer from some form of RLS”. It is typically characterized by leg discomfort or pain that is partially relieved by moving ones legs.

Dr Frankel became interested in testing out CBD for RLS because of it’s complexity and the difficulties that accompany it’s treatment. He started five patients on CBD with RLS. He says they take 4-6mgs a day, under the tongue, and quickly notice a difference. Of these five patients using CBD, three of them had nearly complete cessation of their symptoms and the other two had marked improvement.

Dr Frankel says his patients take the CBD before going to sleep, and some will take it in the morning when they awake. He goes on to say that it is important to consume a product that is full spectrum, containing a variety of cannabinoids and terpenes. “It is never just a one molecule deal”, he says. “It is the enourage effect of all of natures medicine working together, not just CBD. Another study is occurring in Israel regarding CBD and it’s treatment of RLS. ““We are excited to be able to contract with an Israeli CRO to conduct a pre- and a clinical proof-of-concept study for this unmet medical need condition. Israel is in the forefront of clinical cannabinoid research and we are thankful for having this opportunity. RLS is a prevalent condition affecting between 3.9-14.3% of the US population. The available treatments are not very effective and often cause severe side effects,” said George E. Anastassov, MD, DDS, MBA and Chief Executive Officer of AXIM Biotech. “We hope that our proprietary treatment modality utilizing cannabinoids and gabapentin in functional chewing gum will prove to be effective in alleviating the symptoms of RLS.””

CBD And Multiple Sclerosis

The National Institutes of Health have a study regarding multiple sclerosis. To set the scene, it begins by stating:

“Inflammation in the central nervous system (CNS) is a complex process that involves a multitude of molecules and effectors, and it requires the transmigration of blood leukocytes across the blood-brain barrier (BBB) and the activation of resident immune cells. Cannabidiol (CBD), a non-psychotropic cannabinoid constituent of Cannabis sativa, has potent anti-inflammatory and immunosuppressive properties. Yet, how this compound modifies the deleterious effects of inflammation in TMEV-induced demyelinating disease (TMEV-IDD) remains unknown.”

Using a viral model of MS, the study demonstrated that CBD decreases the transmigration of blood leukocytes by downregulating the expression of vascular cell adhesion molecule – 1 (VCAM-1), chemokines (CCL2 and CCL5) and the proinflammatory cytokine IL – 1β, as well as by attenuating the activation of microglia.

CBD was also shown to have long lasting effects  when administered at the time of viral infection. The study concluded by saying, “Together, our findings highlight the anti-inflammatory effects of CBD in this viral model of MS and demonstrate the significant therapeutic potential of this compound for the treatment of pathologies with an inflammatory component.”

This is encouraging to those struggling with Multiple Sclerosis. It also highlights it’s anti-viral properties in general. Knowing that inflammation is the primary cause for dozens of problems people have, helps explain why CBD can be used for so many things. The anti-inflammatory properties were again highlighted throughout this study.

CBD As A Treatment For Schizophrenia

A study in December of 2017 was published by PubMed discussing how effective CBD might be for those suffering from Schizophrenia. The study was done in both animals and humans and indicates that CBD has antipsychotic properties. This is important because not many studies have involved humans.

So, exactly how did they do this? In a double-blind parallel-group trial, “patients with schizophrenia were randomized in a 1:1 ratio to receive CBD (1000 mg/day; N=43) or placebo (N=45) alongside their existing antipsychotic medication. Participants were assessed before and after treatment using the Positive and Negative Syndrome Scale (PANSS), the Brief Assessment of Cognition in Schizophrenia (BACS), the Global Assessment of Functioning scale (GAF), and the improvement and severity scales of the Clinical Global Impressions Scale (CGI-I and CGI-S).”

The results? After 6 weeks of treatment, the CBD group had far lower levels of psychotic symptoms than the placebo group. They were more likely to be rated as improved, and not as severely unwell by the treating clinician. Additionally, although it was not of statistical significance, the patients receiving CBD also showed greater improvement in cognitive performance and functioning than those receiving the placebo.

One of the best reports from this finding were on the side effects. It is stated that “CBD was well tolerated, and rates of adverse events were similar between the CBD and placebo groups.”

The study concluded by saying that these findings suggest benefits for patients with schizophrenia. CBD’s effects do not appear to depend on dopamine receptor antagonism – which means CBD probably represents an entirely new class of treatment for the disorder.

As more studies emerge, it becomes more and more clear that CBD may be the future of medicine. Although this is anecdotal evidence, it suggests that CBD may be an effective treatment for schizophrenia, with no side effects, and in a way no current medications work. That’s pretty exciting!!

CBD may not actually help you count sheep, but it may help you sleep! This article by the Sleep Doctor (Dr. Michael J Breus), explains what he has discovered about CBD.

He explains that it is still a process in understanding exactly how CBD works for calming effects, pain-reducing effects, and anti-inflammatory effects in the body. He says that CBD is known to interact with a variety of receptors, proteins, and chemicals in the brain. He notes that CBD appears to affect a lot in body, including sleep-wake cycles and emotional regulation, to other health issues like inflammation, pain perception, and seizures. The endocannabinoid system makes their own form of cannabinoids. Research is showing that CBD may actually play a role in the growth of new brain cells, a process known as neurogenesis, and also having anti-oxidant and anti-inflammatory abilities.

As for sleep, Dr. Breus explains that CBD works for sleep because of its ability to reduce anxiety which is helpful in reducing sleep difficulties and improving sleep quality. He says that CBD has been shown to reduce insomnia in people who suffer from chronic pain. In small doses, CBD stimulates alertness and reduces daytime sleepiness – enhancing people’s daytime performance. He says that CBD helps reduce REM behavior disorder in people with Parkinson’s Disease. Typically during REM, the body is largely paralyzed, known as REM atonia. With REM behavior disorder, this doesn’t occur, preventing sleepers from resting without moving. He states that CBD may also improve REM sleep abnormalities in people with PTSD.

Dr Breus continues on with the potential benefits of CBD including depression, pain, epilepsy, addiction, neuroprotection and neurodegenerative diseases, cancer, and more. He suggests a dose as small as 10mgs per day – noting that everyone’s body is different. He says that CBD is generally well tolerated by adults, but side effects may include low blood pressure, sleepiness, dry mouth, or lightheadedness.

CBD and diabetes is a growing topic of discussion. Diabetes affects approximately 400 million people worldwide and is responsible for more than 5 million deaths per year. In America alone, over 30 million people have diabetes. It’s important to note the difference in Type 1 and Type 2 diabetes. Type 1 is an autoimmune disease where the body attacks the islets of Langerhans, destroying the insulin producing cells. The body no longer produces any insulin. Type 2 is far more common, and is caused by insufficient insulin being produced. This often slows metabolism and increases blood sugar levels.

Certain organs, the brain, specific glands, connective tissue, and immune cells have endocannabinoids and their receptors. The purpose of this is to always reach homeostasis.

Regardless of Type 1 or Type 2 diabetes, the issue is the person is not getting enough insulin. Cannabinoid receptors, according to this article, have been found present in insulin producing cells, and the activation of the CB1 receptor has been linked to insulin production. A five-year study, released by the American Journal of Medicine showed that people using cannabis had a mean insulin resistance of 1.8, as compared to 2.2 for previous cannabis users, and 2.5 for those who never had used cannabis.

CBD’s use as an anti-inflammatory agent has been documented more than probably any other use for CBD. This is important with diabetes because chronic inflammation can play a role in the development of insulin resistance. Decreasing inflammation can also help the metabolism of the body.

Secondly, this article refers to CBD and its improvement on metabolism. A study in 2013, according to the article, found that cannabis users were found to have lower BMI, despite higher-than-average everyday consumption of calories. This is believed to mean that cannabis increases metabolic rate and helps the body process calories faster and more efficiently.

Finally, it should be noted that most of this research is strictly anecdotal. The evidence shows that diabetics can be greatly helped by CBD, but many Western Medical Doctors are still reluctant to encourage patients to use it.

I had the opportunity to present at the Institute of Cannabis Research (ICR) Conference at Colorado State University.  The ‘Father of Cannabis’ – the most well known researcher for compounds of cannabis, including CBD (Cannabidiol) – Professor Raphael Mechoulam was the keynote speaker.  Researchers from around the world presented their research and industry experts made contacts to further the understanding and applications of this incredible plant.

My presentation: ’That’s Natural ‘Legal Cannabis’ discussed the moral, ethical, and constitutional ‘legality’ of cannabis use, including:

1. Constitutional ‘legality’ of individuals’ freedom to consume plants for foods and medicine – American’s right to life, liberty, and the pursuit of happiness.

2. 10th Amendment protections for States and individuals when the federal government oversteps its bounds and does not protect individuals’ rights.

3. Representatives ‘Oath of Office’ – meant to protect individuals’ natural and constitutional rights – like the right to consume plants for medicines.

4. Practitioners and doctors ‘Hippocratic Oath’ – meant to protect patients and ensure that healers protect health over motives for personal profit.

5. American’s active participation in creating laws, changing laws, nullifying laws (using the 10th Amendment), and breaking laws is what forms moral and ethical guidelines.

6. Stopping human suffering and helping people to heal is a moral and ethical obligation of all individual citizens but especially those that take an Oath of Office to defend the Constitution and the Hippocratic Oath to protect patients.

The Constitution – even from before its writing and ratification – has been a document that has divided government power to protect and preserve the individual rights of the people.  Individual rights – the right to life, liberty, and the pursuit of happiness – as long as you are not infringing on other peoples’ life or property – are unique among Americans.  American citizens enjoy protections in the Constitution – which is the oldest living paper Constitution in the entire world.

America’s form of government is called a ‘constitutional republic’ although many people refer to it as a ‘democracy’.  A unique feature of the government is the separation of powers in government – in the Legislative, Executive, and Judicial branches of government.  Having these three different parts of government – the part that creates laws (Legislative Branch); the part that enacts and enforces laws (Executive Branch); and the part that interest laws (Judicial Branch) – is what is supposed to protect individual’s natural rights.

There is also a difference between ‘legal rights’ and ‘natural rights’.  “Legal” rights are those that are ‘given’ to you by the government.  “Natural” rights are those that are given to you by your Creator.  I believe that it is a natural (moral and ethical) right to consume plant-based medicines.  Just because something is ‘legal’ – in no way does that make it moral or ethical.  I believe it is moral and ethical to stop human suffering.  Cannabis can help stop human suffering.

The Constitution itself says nothing about cannabis – as a matter of fact – that Constitution says nothing about Agriculture, Healthcare, Energy, Social Policy, etc.  Agencies that have been created through laws passed in Congress may be funded by taxpayers – but their functions and actions may not necessarily be Constitutional.  The Constitution is supposed to protect individuals’ life and property.  “Laws” that stop people from being able to access plant-based medicines goes against protecting peoples’ life.  People have a right to be healthy and seek wellness – many government agencies end up interfering in their quest for foods and medicines that can heal them.

Because we have these checks and balances in government at the federal level – it affords Americans the opportunity to change laws that have been created that may have negative consequences for their individual rights.  And, new laws can be created to replace old laws that cannot be changes.  And, worst case – bad laws can be broken using States’ rights.

The 10th Amendment of the Constitution reads:

“The powers not delegated to the United States by the Constitution, nor prohibited by it to the States, are reserved to the States respectively, or to the people.”

When the federal government creates laws that fail to uphold the meaning and intent of the Constitution – State governments can protect their citizens.  Amendment 64 in Colorado effectively did this with the ‘legalization’ of cannabis.  Although this can create a lot of confusion and a ‘gray area’ – ultimately this is protecting individuals’ right to the pursuit of health.  Colorado’s experiment in ‘nullifying’ or ‘ignoring’ federal law is what is giving citizens’ access to new and innovate treatments with cannabis.  Although imperfect, this regulatory framework is proving to be an effective model and is allowing individuals to exercise their Constitutional freedoms.

Our representatives in the Congress and Senate take an ‘Oath of Office’ which reads:

“I do solemnly swear (or affirm) that I will support and defend the Constitution of the United States against all enemies, foreign and domestic; that I will bear true faith and allegiance to the same; that I take this obligation freely, without any mental reservation or purpose of evasion; and that I will well and faithfully discharge the duties of the office on which I am about to enter: So help me God.”

Our representatives are supposed to put our natural and constitutional rights above their personal motives for profit.  Our representatives are charged with protecting us as individuals.

In addition to ‘leaders’ who write laws taking an oath – practitioners and doctors who are charged to help people with their health take the ‘Hippocratic Oath’.  By taking this oath they are swearing themselves to seek help for their patients above and personal motive for profit or gain.  Practitioners and doctors take this oath – and patients trust them to make decisions that are based on compassion – not on profit.

It is a crime that many of the people who create laws and people who write prescriptions are stopping the research and stopping the freedom for people to try and use natural non-psychotropic medicines that can give their body and mind PEACE.  It is against their sworn Oath of Office to protect and defend the Constitution and it is against the Hippocratic Oath taken to protect patients.  It is the citizens’ job to stand up to these unlawful and unethical practices and protect their fellow human beings.  We must hold not only our representatives accountable, but we must also hold doctors and practitioners accountable.  Individuals have a right to use natural, non-psychotropic (does not get you “high”) plants – whether that be hemp, elderberry, or echinacea.

Cannabidiol’s (CBD’s) Impact on Parkinson’s Disease

Over 60,000 Americans are diagnosed with Parkinson’s Disease (PD) each year, while approximately 10,000,000 people worldwide live with the effects of this disease. PD is caused by a loss of brain cells in the substantia nigra, the part of the brain that produces dopamine (a chemical messenger). So, it is no surprise that Cannabidiol (CBD) has been shown to help those with PD find some relief from the symptoms.

This study showed that while there were no adverse effects, and no worsening of motor function, it did decrease the total scores of the Unified Parkinson’s Disease Rating Scale (UPDRS). They claim that:

No adverse effect was observed during the treatment. These preliminary data suggest that CBD may be effective, safe and well tolerated for the treatment of the psychosis in PD.

Another study showed that even though their 119 patients didn’t see any statistically significant difference in UPDRS scores while given CBD, scores on the Parkinson’s Disease Questionaire – 39 test were significantly different than those given a placebo. Their conclusion was as follows:

“Our findings point to a possible effect of CBD in improving quality of life measures in PD patients with no psychiatric comorbidities; however, studies with larger samples and specific objectives are required before definitive conclusions can be drawn.”

A third study from the Movement Disorder Society showed that CBD may be useful in the treatment of motor complications in PD. In this study, it was shown that the long-term impacts of levodopa were not nearly as severe with CBD as without it.

Although more research is likely needed, there is strong evidence to support the premise that CBD does help alleviate some of the symptoms that are most bothersome to those with PD.
